2011-2012 HEALTH FORMS

The Health Forms are now available in the Division Offices, the Nurse's Office or on-line (click on the links below). Every child must have a Health Form submitted annually. This physical expires 1 year from the last day that it is signed and dated by the physician. This form must be signed after May 1st.

These forms must be completed and submitted by Thursday, July 1st 2011.

If the forms are not submitted by 7/1/11, you and your child will not receive subsequent, important mailings (schedules, teacher assignments, etc.) from the Division Offices until the forms are submitted. Please schedule your child's appointment now to avoid complications this summer.
